Am I Eligible?

Case Management

To be eligible for Medicaid Targeted Case management (MTCM), an individual must be enrolled in Medicaid and:

​

•Meet eligibility criteria for one of the target populations outlined in the South Carolina State Plan, and

•Be able to participate in the planning process, or if applicable, a responsible party participating on behalf of the beneficiary, and

•Be a voluntary participant and able to benefit from MTCM services.

 

Anyone can make a referral for MTCM.

You have several options for submitting a referral but please use this freedom of choice form to choose Aging with Flair and call us at (864) 757-9920 or toll free at (877) 285-7465:
 

1. Online using the CLTC Phoenix system: https://phoenix.scdhhs.gov/cltc_referrals/new

OR

  1. 2.Call 1-888-356-2197 and reach someone at the SC Healthy Connections office who will help get your referral to the right place.

OR

3. Send a FAX to 1-803-255-8209 and reach someone
   at the SC Healthy Connections office who will help
   get your referral to the right place.

OR

4.Send a letter to
SCDHHS, MTCM Intake – J7,
P. O. Box 8206,
Columbia, SC 29202-8206

OR

5. Contact Aging with Flair and request free  assistance from one of our Case Managers located throughout the state.

EARLY INTERVENTION Service 

Infants and toddlers from birth to three years may be eligible for BabyNet services if they are developing more slowly in any of the following ways:

 

  1. Learning to crawl and sit

  2. Learning to walk

  3. Height & weight

  4. Learning how to think

  5. Learning to listen & talk

  6. Getting along with others

  7. Doing things on their own

 

Any child who has been professionally diagnosed with one of the conditions listed here is automatically eligible for BabyNet services based on "established risk" according to BabyNet policies and procedures.

 

Older children may qualify for services through SC Department of Disabilities and Special Needs (DDSN). You can contact them at 1-800-289-7012. Aging with Flair staff will be glad to help you with the DDSN referral process.


All children who meet the eligibility criteria will be served regardless of family income.

2. Call 1-877-621-0865 and reach someone in the State BabyNet office who will help get your referral to the right place.

4. Contact Aging with Flair and request free assistance from one of our Early Intervention specialists located throughout the state.

If you are not eligible for BabyNet services for whatever reason, Aging with Flair is here to help. If you are a Medicaid beneficiary, you may qualify for Medicaid Targeted Case Management or we can arrange for substantially similar private services to fit your budget.

Attention Childcare Operators and other Community Organizations!

 

Aging with Flair Interventionists would be pleased to visit your facility to consult and train with your staff.

 

With informed parental consent, we can conduct screenings using the Ages and Stages standard used by BabyNet to facilitate any BabyNet applications. In addition, we will follow-up after the fact to ensure the family has received services, and if not, offer to provide similar services privately!

We all feel overwhelmed from time to time. If you feel you and your family are in constant turmoil, or if you’re always worried about a family member, you have many options:

•Reach out to friends and family for ideas or support.

•Join community education or support groups.

•Consult educational material at your local library.

•Talk to your teachers, clergy, doctor, or public health nurse.

•Contact government agencies where you can often get free professional counseling advice.

•For serious problems such as drug use, suicide threats or breaking the law, concerned family members can contact the child welfare agency, the local social services department or the police force in your community.


We at Aging with Flair, LLC stand ready to help you in your hour of need to meet the challenges associated with child development, adolescence, disability, and eldercare. Our trained and experienced case managers, educators, counsellors, and therapists have been successful in overcoming many problems relating to growing up and aging.

When all other options fail, we are ready to provide as much or as little help as needed. We can put you in touch with appropriate government agencies where you can find help yourself or assist you with navigating the bureaucracy by being your advocate. Our services are often free under various government programs, or if you do not quality, we offer affordable private pay alternatives.

A: Each service provided at Aging with Flair is based on your and your families individual needs. We would be happy to listen to what is going on in your or your loved ones life to help you come to the best decision on what services would meet your needs at this time.  If we are not able to provide you service- we will make sure that you get connected with someone who can!

Q: The language is confusing, how do I know if I meet the criteria for these services?
